Athletic Forms

All student-athletes must submit completed PIAA and UPMC forms before participating in sports. Forms are available on this page or can be picked up in the athletic offices.

No athlete may participate without completed forms on file prior to the start of the season.

Participants and their parent/guardian will also need to complete the  Guidelines and Code of Conduct form. 

All completed forms can be turned into your coach or the EA athletic office.

2025-2026 FREE Sports Physicals - May 16, 2025

Free sports physicals, completed by the school physician, for the 2025-2026 school year will be held on May 16, 2025 during school hours.    

Students athletes and students considering sports, entering 7th through 12th grade, are encouraged to take advantage of this opportunity.  Sports physicals must be completed PRIOR to participating in sports.  

25-26 School Sports Physical Registration

To treat your child, UPMC Athletic Training Services requires the following forms to be completed.  (these forms are included with the UPMC packet above)

  • UPMC Authorization for Release of Protected Health Information
  • UPMC Consent for Treatment and Healthcare Information
  • UPMC Privacy Practices
